Importance of endometrial cytology in diagnosis and therapy of uterine carcinosarcoma

Abstract
Objective : We reviewed the cases of uterine carcinosarcoma diagnosed by preoperative endometrial cytology and biopsy, and evaluated the importance of endometrial cytology in the diagnosis and treatment of uterine carcinosarcoma.
Study Design : We examined the diagnostic accuracy of preoperative endometrial cytology and biopsy in 31 cases with suspected uterine malignancy who were finally diagnosed as having uterine carcinosarcoma. In addition, we reviewed the operative histological diagnosis in the cases that could be diagnosed preoperatively by endometrial cytology as uterine carcinosarcoma.
Results : Carcinosarcoma could be diagnosed by preoperative endometrial cytology in 10 cases, whereas biopsy was accurate in 14 cases. The diagnosis of carcinosarcoma or carcinoma was made by either endometrial cytology or biopsy in 30 of the 31 cases. Moreover, among the 13 cases diagnosed as carcinosarcoma by preoperative endometrial cytology, the operative histological diagnosis was carcinosarcoma in 10 cases and carcinoma in 3 cases.
Conclusion : Carcinosarcoma or carcinoma could be diagnosed preoperatively in almost all of our cases by a combination of endometrial cytology and biopsy. At present, uterine carcinosarcoma is treated in the same manner as uterus body carcinoma. Therefore, it appears possible to determine the appropriate therapy preoperatively by using endometrial cytology and biopsy in combination.
