We report a case with non-convulsive epilepticus (NCSE) manifested by coma. Case: A 51-year-old man was admitted due to multiple trauma caused by a crash and impaired consciousness. He had hemothorax, pelvic fracture, and traumatic subarachnoid hemorrhage. His general condition improved in four days, however, he remained comatose. We could not perform head magnetic resonance imaging immediately because he had undergone an orthopedic surgery. Therefore, we performed continuous electroencephalography (EEG) instead, and it revealed that his condition was NCSE. The administration of antiepileptic drugs successfully improved his consciousness. It is known that the delay of the intervention negatively affects the clinical course of NCSE. Recently, more handy and feasible EEG devices have been developed and commonly used in daily practice. Using those devices, we should measure EEG as early as possible to identify NCSE and other treatable consciousness disorders.
