Background: Infants whose mothers have chronic abruption-oligohydramnios sequence(CAOS)have a poor outcome, but there are very few cohort studies from neonatologist.
Objective: To investigate the outcome of respiratory and growth and development in infants born from mothers complicated with CAOS.
Methods: Medical records of maternal and neonatal factors, and data on the neonatal short-term outcomes; the rate of chronic lung diseases, home oxygen therapy(HOT), death, death due to cor palmonale, and long-term outcomes; height SD, weight SD, and total DQ values at 18 months corrected chronological age(c-CA)were examined.
Results: The neonates born from mothers with CAOS group had significantly high rate of chronic lung disease(94.1% vs 71.5%), death due to cor palmonale(17.6% vs 0.6%), and HOT or death due to cor palmonale(58.8% vs 19.6%)compared to the non-CAOS group. There were no significant differences in height SD, weight SD, or total DQ values at 18 months c-CA. After adjustment for background factors, the CAOS group was at significantly higher risk than the non-CAOS group for HOT or death due to cor palmonale(odds ratio[95% CI] 3.85[1.27, 11.7]).
Conclusion: Infants born from mothers with CAOS are expected to have a poor short-term respiratory prognosis but their growth and development are not significantly different from those of infants in the non-CAOS group. It is necessary to develop strategies for the prevention and treatment of chronic lung disease exacerbated by CAOS and to study longer term outcomes.
