Duodenal atresia is sometimes accompanied by pancreaticobiliary anomalies such as pancreas divisum. We report the case of a 2-year-old boy who underwent accessory sphincteroplasty owing to recurrent pancreatitis associated with pancreas divisum. The patient had undergone radical operation for duodenal atresia with the membrane on day 1 after birth. He developed recurrent pancreatitis at two years of age. By detailed examination with ERCP and MRCP, we diagnosed him as having complete pancreas divisum and a Y-shaped opening of the common bile duct. We suggested that pancreatitis was caused by a relative stenosis of the minor papilla associated with pancreas divisum. Therefore, accessory sphincteroplasty was performed. The postoperative course was uneventful and the patient did not show any sign of recurrence within 25 months after the surgery. We consider that accessory sphincteroplasty is the first choice for recurrent pancreatitis associated with pancreas divisum.
