A female baby, who had been diagnosed as having severe right-sided congenital diaphragmatic hernia (CDH) by fetal MRI, was born by scheduled Cesarean section at 37 weeks of gestation. She received respiratory control with gentle ventilation just after birth and her general condition was stabilized early. Two days after birth, her CDH was surgically repaired. A right posterolateral diaphragmatic defect within the hernia sac was found. In the sac, the right lobe of the liver, small bowel and colon were herniated. The hernia sac was excised and the diaphragmatic defect was repaired using a Gore-Tex® sheet. Her postoperative course was relatively uneventful. Histopathologic examination revealed ectopic liver tissue in the hernia sac. Although ectopic liver tissue in the sac of CDH was rarely reported, it may be a useful finding for understanding the pathogenesis of CDH.
