A four-day-old girl with a congenital imperforate hymen and an abdominal mass was referred to our hospital. The mass was detected by fetal ultrasonography at 39 weeks of gestation and suspected to be an ovarian cyst. On admission, we found an imperforate hymen and hydrocolpos by vulvar examination and ultrasonography. Owing to a giant hydrocolpos, hymenotomy was performed, and a white turbid fluid of 95 ml volume was drained. After the procedure, the hydrocolpos showed improvement. An imperforate hymen with symptoms in the neonatal period is rare, but occasionally, the secretion accumulates in the vagina owing to maternal female hormones. From our experience, a simple hymenotomy is effective for cases with an imperforate hymen associated with giant hydrocolpos.
