We report a case in which therapeutic Lipiodol lymphangiography was effective for the management of refractory lymphatic leakage following nephrectomy for nephroblastoma. A 2-year-old girl developed chylous drainage after nephrectomy for nephroblastoma. Conservative management, including the administration of a medium-chain triglyceride formula and octreotide, showed limited improvement. On postoperative day 44, therapeutic lymphangiography was performed by injecting Lipiodol into the bilateral inguinal lymph nodes, without any complications. The drainage volume decreased significantly from the day after the procedure, and no recurrence was observed during the follow-up. The findings in this case suggest that therapeutic Lipiodol lymphangiography is a safe and effective therapeutic option for refractory lymphatic leakage after the resection of solid pediatric abdominal tumors.
