This study was designed to determine the clinical response to Hydroxyethyl Starch solution as a plasma substitutes.
The results indicate that the HES solution was well tolerated in almost all patients, particularly in geriatric surgery.
1. From five hundred to 1500ml. of HES solution were administered intravenously to 12 preoperative patients including four cases of over 60 years old. Decrease of Ht., Hb., RBC, and WBC, which were supposed to be due to hemodilution, were observed. Increase of serum albumin and decrease of serum globulin besides decrease of serum protein were observed in all cases. Being yet not significant, it was supposed that HES solution might improve liver and kidney function. Arterial pH remained normal. More over, no side effect was observed.
2. From five hundred to 2000ml. of HES solution were administered intravenously during surgery under general anesthesia to 45 patients including 11 of over 60 years old. Blood loss during surgery remained non-significant with that of control group. Three of 45 cases showed less 30% decrease of blood pressure during surgery, and others remained favorable level after surgery as well. Central venous pressure was slightly increased in a few cases. No circulatory and respiratory trouble was encountered even in an emergent and aged patient during surgery. Urinary output in postoperative period was favorable and all cases showed uneventful postoperative course.
