V-mammoplasty is step 2-② procedure of OPBCS volume displacement. It is used for patients with cancer lesions on the inner-lower quadrant area of a large and/or ptotic breast in Western countries. We here introduce two techniques for Japanese patients : V-mammoplasty and V-rotation mammoplasty. The latter one includes some modifications of V-mammoplasty for patients with small or non-ptotic breasts.
I. V-mammoplasty : This procedure involves excising a pyramidal section of the gland, with its base located in the inframammary fold and apex at the border of the areola. The section is removed en block, including the skin attached to the gland down to the pectoralis fascial. It can be used for patty breast because we does not undermine the skin. The inframammary fold is incised from the resection site to the anterior axillary line. The lower pole of the breast is removed from the pectoralis muscle and is transferred medially to fill the defect.
II. V-rotation mammoplasty : This procedure is a modified technique of V-mammoplasty. It is useful for patients with non-ptotic and small to medium size breasts. The skin glandular-fatty graft is composed of a lower part of the breast and additional skin. Fatty tissue of the caudal part of the inframammary line is rotated to fill the defect of the inner part of the breast.
On both techniques, the postoperative radial scar on the inner-lower quadrant area and curved one on the inframammary line are made.
