Background:Cytopenias are common hematological abnormalities seen among HIV-infected patients. These abnormalities could potentially cause serious clinical implications. Published literature on the prevalence of cytopenias among HIV-infected patients in Bali are limited. Therefore we conducted this study to assess the hematological profile and to determine the prevalence of cytopenias among ART-naive HIV-infected patients and HIV-infected patients on ART in Bali.
Methods: This is a descriptive cross-sectional study using data extracted from medical records of HIV-infected patients Wangaya Hospital Denpasar Bali and Sanjiwani Hospital Gianyar Bali. A total of 243 medical records of HIV infected patients between 2009 -2017 were included in this study. Study subjects were divided into into two groups; ART-naive patients and patients on ART. Hematological parameters (total WBC, RBC,Hgb, MCV, MCH, MCHC,and Plt) were documented in each group, as was the presence of any cytopenias.
Results:Most of subjects in this study were predominantly male (62.6%) and were in highly reproductive age group (18-40 years). There were significant differences in total RBC, Hgb, MCV, and MCH between ART-naive patients and patients on ART (p < 0.05). Prevalence of anemia, leucopenia and thrombocytopenia in ART-naive patients were 60.8%, 8.2% and 9.4%, while prevalence of anemia, leucopenia and thrombocytopenia in patients on ART were 40,3%, 6,9%, dan 4,3%. There was a significant difference in prevalence of anemia between ART-naive patients and patients on ART (60.8% vs 40.3%; p < 0.05).
Conclusions: Anemia was the most common cytopenia seen in this study. The prevalence of anemia in ART-naive patients was higher than in patients on ART.
