Updated Banff classification 2013 encompasses revised scheme of antibody mediated rejection (ABMR). The modified diagnostic criteria of ABMR consists of DSA-positivity, characteristic histological features of acute and chronic ABMR, and DSA-induced endothelial cell activation which is manifested by either C4d positivity, microvascular inflammation or expression of activated endothelial gene transcripts. Other revised criteria include histological definition of transplant glomerulitis (g) and transplant glomerulopathy (tg). Endothelial arteritis by itself does not warrant the diagnosis of acute T-cell mediated rejection. With the inclusion of gene expression data relevant to ABMR, histopathology-based criteria of the Banff classification is dramatically shifting to “Seeing the Unseen” scheme.
