Selection of right or left retroperitoneoscopic donor nephrectomy with multiple renal arteries

Abstract

We performed 12 living kidney transplants between 2012 and 2013. Three of the donors had two left renal arteries. The renal artery branched off at an early stage, and all cases required angioplasty of three vessels after retroperitoneoscopic donor nephrectomy. In one case, left nephrectomy and transplantation were done by forming three vessels into one. In the other two cases right nephrectomy was performed and the vein was extended using a saphenous vein graft. All three renal grafts showed good function, and there were no complications such as bleeding and thrombus. Postoperative urinary output was lower in the case where artery formation had been performed. When a donor has multiple left renal arteries, and angioplasty of three vessels is needed to form an early branch, it is a useful to select donor right nephrectomy and extend the renal vein using a saphenous vein graft.
