Immune reconstitution inflammatory syndrome in polyomavirus BK nephropathy

Abstract

Screening with polymerase chain reaction in plasma and urine cytology help early detection of BK viral reactivation in kidney transplant patients. Those screening strategy and pre-emptive reduction of immunosuppression (IS) reduced the incidence of diagnostic Polyomavirus BK associated nephropathy (BKVN). However, natural course of BKVN is still unclear, and serum creatinine level does not decrease soon, or often increase after the reduction of IS. In this setting, we need to consider two possibilities;acute rejection caused by reduction of IS, or immune reconstitution inflammatory syndrome (IRIS) after the recovery of immune function. We hereby discuss the role of IRIS in resolving BKVN by analysis of our re-biopsy samples.
