Kidney transplantation for long-term dialysis patients

Abstract

It is common for kidney transplantation (KTx) candidates to receive long-term dialysis, because waiting time for deceased donor KTx is over 15 years in Japan. We evaluated the influence of dialysis duration on the outcome of KTx. We retrospectively analyzed the patients who underwent KTx in our institution from 2009 to 2015. We divided the patients into two groups, group A:preemptive KTx or the patients who received the dialysis therapy for less than 10 years before KTx, and group B:the patients who received the dialysis therapy for 10 years or more before KTx. We compared their demographics features, clinical data, and survival outcomes between two groups. 59 patients were evaluated (Group A, n=52;group B, n=7). The median age of recipients in group A was 42.5 and 62 years old in group B (p=0.0959). Two cases in group B (29%) were deceased donor KTx recipients. Cytomegalovirus-negative recipients from donors positive for the virus were 12 cases (23%) in group A, and none in group B. There were no differences in graft function and graft survival between groups. The ureter anastomotic time was 32 min in group A and 40min in group B. This study showed that the duration of dialysis did not affect the clinical outcome of KTx in selected patients.
