The purpose of this retrospective study was to compare mini-PCNL with and without a suction system to determine the usefulness of vaccume-assisted percutaneous nephrolithotripsy (PCNL). Sixty patients who underwent mini-PCNL for renal stones at the Osaka General Medical Center were divided into PCNL va (＋) group (n＝32) : patients who were operated with a suction sheath and PCNL va (－) group (n＝28) : patients who were operated without suction sheath. The stone free rate (SFR) in the first operative day was 68.8％ in the PCNL va (＋) group, which was significantly higher than that of the PCNL va (－) group (28.6％) (p＜0.01). Complication rates were 6.3％ in the PCNL va (＋) group and 17.9％ in the PCNL (－) group. Vaccume-assisted mini-PCNL was considered to be an effective method for reducing complications and achieving a high SFR.
