Some ideas for performing transnasal endoscopy smoothly in stomach cancer screenings at health check-up centers

Abstract
Transnasal endoscopy (TNE) has been gaining popularity in Japan as a safe and painless upper GI procedure, whereas there are actually some problems resulting from the ultrathin diameter of the endoscope, because various functions are sacrificed for the sake of thinness. We consider that it is necessary to come up with some ideas to perform TNE smoothly for many examinees within a limited time frame. We established the TNE center in April 2004. We are using a DVD for informed consent, 150 ml of dimethicone water in the preparation, a dedicated report format for the TNE, and so on. We can perform 24-28 examinations from 9 a.m.to 1 p.m. using 2 endoscopic systems. At present, all TNEs at our hospital are done at this center. From April 2006 to March 2010, 9834 patients were screened for stomach cancer. During this period, we detected 21 cases of stomach cancer, with a detection rate of 0.21%; 16 (76.8%) of them were in the early stage, and the false negative rate was 28.6% . These results are similar to those described in previous reports.
Many endoscopists will introduce TNE into their UGI examination from now on, as TNE has been well accepted by patients. This time, we report some of our ideas on how to overcome the problems of the ultrathin endoscope for smooth examinations in screening, as well as the results of such examinations.
