Analysis of reasons for interruption and non-implementation of home hemodialysis

Abstract

At our hospital, we introduced home hemodialysis (HHD) in February 2012, and have managed 4 patients in the past. Among them, there were sessions where the scheduled dialysis could not start, or the dialysis was started but could not be completed and ended midway. We retrospectively analyzed the number and causes of interrupted and non-implementation sessions. Of the 5855 sessions, 92 sessions (1.6%) were interrupted between February 2012 and March 2021. The most common cause was circuit coagulation and circuit pressure increase in 44 sessions (48%), followed by mispunctures in 27 sessions (29%). In the early stage of introduction, interruptions due to mispunctures or operation mistakes were common, but these no longer occurred after about 2 years. Seven interruptions related to the dialysis machine have occurred so far but were managed early and treatment was not significantly affected. HHD interruptions and non-implementation not only increase the mental and physical burden on patients, but also cause major medical economic issues; therefore, regular audits are necessary at all facilities.
