Total hip arthroplasty (THA) for dysplastic hip osteoarthritis requires an acetabular bone grafting to obtain adequate support for the acetabular socket. We report our experience with L-shaped bone grafting from the femoral head, after Radojevic et al., in primary THA for dysplastic hip osteoarthritis. In this study, 28 joints in 23 cases were followed for more than five years after operation (average follow-up : 79 months) . All the grafted bone was fused to the acetabulum within 12 months without collapse, although non-weight-bearing parts of the grafted bone had been absorbed and remodelled. A radiolucent zone wider than 2 mm was detected in one joint at zone 1, two at zone 2, and five at zone 3. There was no socket migration. The L-shaped bone grafting of the femoral head in THA for dysplastic hip osteoarthritis was shown to have good medium-term results with good initial fixation and no collapse which resulted in low rates of aseptic loosening.
