We report a rare case of iliopectineal bursitis after total hip replacement. A 61-year-old female visited our hospital complaining of swelling in her left lower limb 6 years after left total hip replace-ment. We observed enlargement of the Iliopectineal bursa that compressed the external iliac vein, and the patient underwent bursectomy. Polyethylene wear debris was found in the bursa. It was considered that the iliopectineal bursitis was caused by this polyethylene wear debris that had been generated in the hip joint and translocated to the bursa through the communication channel between the hip joint and the bursa. During follow-up, no recurrence has been observed at 1 year and 10 months after the bursectomy. Thus, bursectomy was considered an effective treatment method for iliopectineal bursitis after total hip replacement.
