Abstract
Recently high-power output diode laser has been developed, since its advantages as small unit size, true portability, and low cost, the diode laser is expected to come into wide use. In this study, we evaluated the ability of the diode laser in the treatment of superficial bladder pathology. The medical diode laser system (DIOMED25, Diomed Company, UK) was used with 25 W at 805nm. The applied output power was 5 W and was delivered to the tissue through a flexible silica glass fiber. A excised fresh swine bladder was used. The irradiation was performed in the non-contact mode with a spot diameter of 2mm. To assess the enhancement effect of indocyanine green (ICG), ICG solution (1mg/ml, 0.5ml) or normal saline was injected submucosally immediately before the irradiation. The macroscopic appearance of the bladder surface was recorded. The specimen was kept in 10%formalin for histological consideration. When applied to the resected swine bladder without ICG pretreatment, the diode laser only minimally coagulated mucosal surface in 3 minutes. With ICG preinjection, vaporization and coagulation of the mucosa was obvious within 10 seconds and the whole mucosal layer was vaporized in 60 seconds. When irradiation surface seemed green, it reached submucosal layer. When white, irradiation surface reached muscle layer. Our results suggested that the diode laser can be used to vaporize and coagulate the superficial pathology of urinary bladder and ICG submucosal injection make selective and safer treatment possible.
