「安全・安心」への道標 : Root Cause Analysis(FM-2 基調講演)

抄録

Anyone who uses a product cannot see the safety of it, even if its supplier has strict confidence in its engineering safety and reliability. Confidence of the safety is depended on the trust for its supplier who can present actions for fairness and integrity. Root Cause Analysis called"RCA"which had developed in USA may be applicable for their communication. RCA introduce corrective actions in management process cooperatively to prevent reoccurrence of the problem by finding cultural issue existed in organizations or stakeholders implicitly. The Thinking way of RCA is indispensable and effective to make way marks for the safety society with no worry if proper understandings for RCA spread in our society. Outline of RCA may be introduced including: special features effectiveness for improving integrity of organizations, and barriers in Japanese culture to get the effectiveness.
