In recent years, many dentists have been performing implant placement procedures, but how many are seriously concerned about the possibility of peri-implantitis in the future? Peri-implantitis is defined as an inflammatory condition caused by bacterial biofilms. These peri-implant biofilms with inflammation can spread further infection around the implant to susceptible hosts. Still, the formation of the biofilms is affected by the local environment and is very similar to the bacterial flora of the remaining natural teeth. Therefore, patients with untreated periodontal disease, residual deep periodontal pockets, and poor oral hygiene are at risk for premature progression of peri-implantitis. Thus, patients with periodontal disease should have periodontal treatment completed before implant placement to improve residual pockets that cause bleeding during probing and to remove subgingival defects.
This article discusses from the standpoint of a periodontist how to treat severe periodontitis with deep periodontal pockets, horizontal bone defects, and vertical defects to prevent peri-implantitis through clinical cases.
