Abstract
Reports of radiotherapy for extrahepatic metastases are rare except for bone metastases. We report two effective cases of radiotherapy for metastases in the mediastinum and the chest wall from hepatocellular carcinoma (HCC). Case 1: A 70-year-old man underwent extended anterior segmentectomy of the liver for HCC with portal vein tumor thrombus (PVTT). Mediastinal lymph node metastases were appeared at 6 months after operation. By radiotherapy, lymph node metastases shrank, and there was no relapse over 1 year. The patient succumbed to brain metastasis at 32 months after operation. Case 2: A 74-year-old man underwent extended left lobectomy of the liver for HCC with PVTT. At 10 months after operation, metastasic lesions in the anterior mediastinum were appeared, and the patient underwent the resection of the metastatic lesions. After the operation, the subcutaneuos metastases at the right anterior region of the chest wall were repeatedly appeared. The patient received radiotherapy, and subcutaneuos metastases shrank. The patient is still alive at 3 years after the initial operation. In our 2 cases, radiotherapy gave the good local control of soft tissue metastases of HCC.
