Abstract
We report a case of primary cutaneous aspergillosis in an 81-year-old Japanese female. She was diagnosed as having B-CLL and AIHA, after undergoing treatment with anticancer drugs and corticosteroid. Granulocyte colony-stimulating factor (G-CSF) was also used for the treatment of B-CLL on the deltoid region. She developed a painful erythema accompanied by bulla on the left deltoid region. The erythema spread and developed into an ulcer measuring 14 cm×6 cm in size. Histologically numerous hyphal elements were seen with neutrophils in the lower dermis. The hyphae were septate and observed to branch at acute angles. Aspergillus fumigatus was isolated from this lesion. The patient was successfully treated with both local and systemic antimycotic agents and by surgery. The direct inoculation of the fungus by the subcutaneous injection of G-CSF was thus suggested to be the cause of the diseage.
