Coil Embolization of a Cerebral Artery Aneurysm via the Right Brachial Artery Using a Combination of a 6 Fr Simmons-Type Guiding Sheath and a 6 Fr Intermediate Catheter

Abstract

Objective: We report two cases of cerebral aneurysm coil embolization using a 6 Fr Simmons-type guiding sheath (SGS) and a 6 Fr inner guiding catheter (IGC) via the upper right arm.

Case Presentations: The first case was a 67-year-old man with an unruptured cerebral aneurysm (right internal carotid artery-posterior communicating artery bifurcation). The second case was a 74-year-old woman with a ruptured anterior communicating artery aneurysm. Due to severe arteriosclerosis and high tortuosity of the aorta, the right transbrachial approach was selected. In both cases, the angle of the bifurcation between the right subclavian artery and the right common carotid artery (CCA) was steep. Therefore, after the tip of the 6 Fr SGS was placed in the right CCA, a 6 Fr IGC was inserted into the SGS and the tip was placed in the right internal carotid artery. Endovascular coil embolization was performed successfully with this procedure.

Conclusion: Our procedure is a useful and safe method for patients who have a cerebral aneurysm and have difficulties in the application of the transfemoral approach.
