Objective: We report a case of coil embolization for unruptured paraclinoid carotid artery aneurysm that caused coil protrusion and delayed distal coil migration resulting in ischemic complications.
Case Presentation: A 57-year-old woman with a history of subarachnoid hemorrhage underwent coil embolization for a left unruptured internal carotid artery aneurysm, and coil protrusion was observed in the parent artery, but the acute phase passed without thromboembolism. Two weeks later, she developed total aphasia and right hemiparesis. Diagnostic imaging revealed distal coil migration to the left middle cerebral artery territory, and the symptoms were improved by the removal of coil using a micro basket.
Conclusion: Coil protrusion on the parent artery during coil embolization may cause delayed coil migration,even if it appears stable. Adjunctive anti-thrombotic therapy or additional stenting may be recommended.
