Objective: We present a case report where we performed coil embolization and angioplasty for rerupture and hematoma formation of ruptured left middle cerebral artery (MCA) aneurysm with cerebral vasospasm.
Case Presentation: A 35-year-old woman felt exhaustion. After 5 days, she had verbal disorder and came to the hospital. Brain CT showed subarachnoid hemorrhage with intracranial hematoma of left temporal lobe. CTA showed left MCA aneurysm and cerebral vasospasm, but cerebral blood flow (CBF) was normal except for the area of intracranial hematoma on CT perfusion. We diagnosed rerupture and hematoma formation and performed coil embolization immediately. We also performed angioplasty because of being apprehensive that cerebral vasospasm would worsen. Her neurological symptoms were getting better, and she was transferred to a rehabilitation hospital after treatment of cerebral vasospasm.
Conclusion: Coil embolization can be an aggressive treatment for rerupture and hematoma formation of ruptured cerebral aneurysm with cerebral vasospasm.
