The eye represents a unique extension of the central nervous system that permits noninvasive vis ualization of systemic metabolic disturbances, including microvascular dysfunction and oxidative str ess. In diabetic retinopathy, cumulative glycemic indices alone are insufficient to explain tissue inj ury; rather, glucose variability induces mitochondrial overproduction of reactive oxygen species, lea ding to endothelial damage and capillary dropout. Strict dietary modulation of glycemic excursions is therefore critical. Similarly, many cases of glaucoma are increasingly linked to optic nerve isch emia and endothelial dysfunction rather than intraocular pressure alone, and enhancement of nitric oxide bioavailability, including L-arginine supplementation, may improve ocular perfusion. Antioxid ative and anti-glycation strategies targeting advanced glycation end products, as well as anti-inflam matory approaches for retinal degeneration, are also discussed. These ophthalmic nutritional strategi es conceptually parallel blood–brain barrier protection and neuronal resilience, suggesting an integr ated translational framework for future psychiatric neuroprotection.
