A Case of Large True Cyst of the Pancreas

Abstract
A 55-year-old woman was admitted to our hospital for the discomfort of the left side of the abdomen. Abdominal US and CT showed a large cyst with the size of 15×12×9cm, formed a simple locule, in the body and tail of the pancreas. Nodules and calcification were not recognized in the wall of the cyst. ERP showed communication between pancreatic duct and the cyst. We performed endoscopic naso-pancreatic drainage of the cyst in order to examine cytology. The result of cytological examination was class II. Drainage was not continued because of severe abdominal pain. From these findings we suspected the cyst as pseudocyst and performed pancreatosplenectomy. Epithelial cells were seen inside the cyst histopathologically, and the cyst was diagnosed as a true cyst, which suggested retention cyst or mucinous cystadenoma with low grade atypia. We finally diagnosed the cyst as mucinous cystadenoma with low grade atypia due to its large volume.
