The Relationship Between Emotional States and Functional Independence According to Differences in Cognitive Strategies among Patients in a Comprehensive Rehabilitation Unit

Abstract

Purpose: This study aimed to clarify the relationship between emotional states and functional independence according to differences in cognitive strategies and to explore the potential for rehabilitation support tailored to these strategies.

Methods: Twenty-nine patients admitted to a comprehensive rehabilitation unit were included. The Cognitive Strategy Scale was assessed at admission, and the Japanese version of the Positive and Negative Affect Schedule (PANAS) and the Functional Independence Measure (FIM) were assessed at admission and discharge. Based on cognitive stratefy, participants were classified into optimism and pessimism groups. Between-and within-group comparisons were conducted, and correlations between PANAS and FIM were analyzed. In addition, between-group comparisons of the length of hospital stay and correlations with PANAS and FIM were examined.

Results: The optimistic group exhibited higher levels of positive affect and lower levels of negative affect at admission, with no significant changes between admission and discharge. In contrast, the pessimistic group maintained elevated levels of negative affect throughout. FIM scores at discharge were significantly higher in the optimistic group than in the pessimistic group, and the length of hospital stay was shorter in the optimistic group.

Conclusion: Patients with an optimistic cognitive coping style tended to maintain positive emotions, demonstrate greater improvements in functional independence, and achieve earlier discharge home.
