Abstract
In virture of a recent improvement of hemodialysis technique, hemodialysis patients can live longer and their aging can lead to have various complications. This paper presents a recent experience with successful surgical treatment for impending rupture of abdominal aortic aneurysm in a hemodyalysis patient is reported.
A 67-year-old man who had been on hemodialysis since 1982 was diagnosed as having an aortic aneurysm in 1985. He had been ovserved without operation, but he was operated because of impending rupture. The aneurysm was replaced by a Y-shaped Gore-Tex graft. After surgery the patient was treated by pulmonary capillary wedge pressure measurement using a Swan-Ganz catheter and early hemodialysis using nafamostat mesilate.
If hemodialysis patient is operated on, severe management of water balance during and after surgery and early postoperative hemodialysis are important. For that, Swan-Ganz catherter and nafamostat mesilate, an anticoagulant with a shorter duration of action, are very useful in the early postoperative period.
