Abstract
A case of primary cystic duct carcinoid is reported with some discussion about 21 case-studies reported in Japan. A 70-year-old woman was admitted to the hospital because of epigastric to right hypochondrial pain lasting for about 2 months. Ultrasonography, DIC, CT, and ERCP indicated a suspecion of confluence stone, and operation was performed. During the operation, the tumor was palpable in the confluence point of the three ducts, and it was removed. The tumor was proved to be a primary cystic duct carcinoma on pathological investigation of frozen section, then resection of the common bile duct, gallbladder with liver bed and hepaticojejunostomy by Roux-Y were performed. Diploid pattern was showed in the flow cytometry test. The concentrations of serotonin in plasma and 5 HIAA in urine after the operation were whithin the normal range. However, the patient experienced multiple hepatic metastases and is undergoing chemotherapy with CDDP and other anticancer agents.
