A case of Morgagni hernia containing the omentum alone is reported with a review of the literature about the diagnosis and surgical approach.
A 74-year-old obese woman was referred to the hospital because of exertional dyspnea. Rentgenograms of the chest showed a fist-sized tumor at the right cardiophrenic angle in contract with the anterior chest wall. The tumor was well bordered and almost homogeneous. Barium enema examination revealed that the transverse colon was pulled upwards. Computed tomograms showed the fat-density tumor in the right side of the chest, and it continued to the abdominal cavity. A median sternotomy was employed for the surgical approach. A hernia was found at the foramen of Morgagni with a sac containing the omentumn alone. From our experience, it is concluded that the computed tomography is useful in order to obtain the diagnosis, and the median sternotomy approach seems to be appropriate.
