Abstract
Malignant jejunocolic fistulae resulting from colon cancer are rare. This report describes a case of jejunocolic fistula due to a transverse colon cancer in a 49-year-old man complaining of diarrhea. Barium enema, upper gastrointestinal series and colonoscopicfiber showed a malignant fistula between the jejunum and transverese colon. At exploratory lapalotomy, a fist sized tumor was nestled in the left transverse colon and directly involved the distal jejunum 10 cm from Treitz's ligament. No liver metastasis and peritoneal dissemination were seen. Resection of the transverse colon in continuity with the fistula and jejunum was performed. Final pathological report revealed a moderately differentiated adenocarcinoma without nodal metastasis [si n(-) P0 H0 stage III]. Seven cases reports of jejunocolic fistula due to a colon cancer containing this case were seen in the Japanese literature. There were no lymphnode metastasis, peritoneal dissemination and liver metastasis in these cases. It is concluded that these colon cancers forming fistula have low grade of biological malignant potential.
