Abstract

A case of omentovolvulus causing by inguinal hernia is reported on, with a review of 70 such cases seen in the Japanese literature.
A 40-year-old man was seen at the hospital because of lower abdominal pain. The patient had been having a right inguinal hernia since 3 years before, but the prolapse of the hernia disappeared 2 days before abdominal discomfort developed. He had tenderness over the lower abdomen with irritative symptom of the peritoneum, which was felt strongest on the right side. Laparotomy was performed under a diagnosis of acute appendicitis. During surgery it was found that the greater omentum, adhering with the right internal inguinal ring, anterior abdominal wall, and anterior wall of the rectum, included tortions at 2 sites causing circulatory failure. The tortions of the omentum were excised and radical operation for inguinal hernia was carried out. Postoperative course was uneventful and the patient was discharged from the hospital.
