Abstract
Omental panniculitis is a very rare disease, and we seldom see the report in the literature. Here a case of omental panniculitis, which was histologically found after operation under a preoperative diagnosis of omental tumor, is described. A 58-year-old man was admitted to the hspital because of a mass at the upper quadrant. There was a history that the had been kicked his abdomen by a horse at the age of 4. The 14×10 cm mass extending over right hypochondrial to umbilical region was palpated, which could be moved to the left upper quadrant manually. Abdominal CT revealed a well-defined tumor consisting of solid and cystic parts, and celiac angiography demonstrated a dilatation of right gastroepiploic artery without the irregularity and encasement. The patient was diagnosed as having a greater omentum tumor. Resection of the tumor and partial gastrectomy were carried out. The resected tumor was 10×10×12 cm in size, encapsulated with smooth membrane, adhered to the serosa of the stomach, and consisted with both solid and cystic portions. Microscopic findings showed omental panniculitis with inflammatory cell infiltration and proliferation of fibroblasts. Clear etiology is unknown, but his experiance of being kicked by a horse in the childhood probably caused this omental panniculitis.
