This paper describes a case of squamous cell carcinoma of the pancreas presenting with a peculiar manifestation with a review of 10 cases seen in the Japanese literature. A 61-year-old male was admitted to the hospital because of a left epigastric pain. A low density area in the spleen was found at CT. He lapsed into shock state due to massive anal bleeding. Emergency operation was performed and a tumor invading the spleen, colon, diaphragma and retroperitoneum was found. En-bloc resection of the spleen, tail of the pancreas and partial colon was performed. The tumor was histologically diagnosed as squamous cell carcinoma of the pancreas origin.
At preoperative MRI the pancreatic tumor could not be found, but the tumor in the spleen was visualized as a low intensity lesion at T1 emphatic MRI and as a high intensity lesion at T2 emphatic MRI. It indicated the necrotized center of the carcinoma in the spleen.
