Abstract
We demonstrated a case of giant gastric stromal tumor which indicated an extramural growth. A 77year-old man was admitted to the hospital because of abdominal distention. Abdominal CT scanning showed a cystic tumor of the upper abdomen and an angiography revealed the tumor was fed by the splenic artery. During operation the tumor originated from the stomach and showed an extramural growth. Neither distant metastasis nor lymph node metastasis was encountered. The gastric mass was extirpated associated with a partial resection of the stomach and a distal pancreatectomy. No obvious remnant tumor was seen. The tumor was 21×20 cm in size and 2.4 kg in weight. Histopathologic examination indicated leiomyosarcoma of the stomach. Further immunochemical staining showed negative for myogenic and neurogenic markers but positive for CD 34. Finally this tumor was diagnosed as uncommitted type of gastric stromal tumor. Rapid growth, containing necrosis, dense cellarity, and many mitosis of the tumor indicated clinically malignant, but in a new pathological entity, the trait of the stromal tumor is unclear. We need further accumulation of clinical cases as well as immunochemical studies for this disease.
