The anterior clinoid process is located at the medial end of the sphenoid ridge, supported by the sphenoid ridge from the lateral side and optic strut from the caudal side, corresponding to a part of the optic canal. Anterior clinoidectomy is a useful technique for obtaining a wide surgical field when securing the anterior bend of the internal carotid artery during clipping of internal carotid artery aneurysms. The step-by-step procedure for the removal of the process was based on the microsurgical anatomy results to avoid thermal and mechanical damages to the optic nerve and internal carotid artery. In this study, the extradural anterior clinoidectomy technique is introduced based on the assumption of neck clipping of paraclinoid aneurysms. Steady stepwise anterior clinoidectomy is predictably effective in neck clipping of paraclinoid aneurysms and for educational purpose, whilst avoiding complications.
