A 76-year-old man presented with swelling and a mass(2 cm)on his left upper eyelid. Five months before his first visit to our department, he was diagnosed with mantle cell lymphoma that was refractory to chemotherapy. An eyelid tumor biopsy revealed metastasis of his mantle cell lymphoma. The tumor rapidly increased in size resulting in loss of the light reflex. We expected the tumor to spread to the orbit, and in fact, it metastasized from the upper eyelid to the orbit within a few days, and head imaging tests revealed compressive optic neuropathy. Chemoradiation therapy led to reduction in the size of the eyelid tumor with an improved light reflex. It is possible to make the compressive optic neuropathy, and early treatment is important to avoid rapid enlargement of a mantle cell lymphoma, which is invariably a fast-growing tumor.
