An 80-year-old woman presented with right lower abdomen erythema, noted 2 years before presentation. Physical examination revealed erythema (size 15×15 cm) and two erythematous tumors (size 6×4 cm and 2.2×2 cm) on it. Based on skin biopsy results, the patient was diagnosed with extramammary Paget’s disease (EMPD). There was no lesion in the axilla, vulva, or anus. Computed tomography revealed no metastasis. Local excision with a 10-mm margin and bilateral inguinal sentinel lymph node biopsy were performed. Histologically, malignant cells with abundant pale cytoplasm were observed in the epidermis and dermis. No evidence of recurrence has been noted until 1 year postoperatively.
It is hypothesized that EMPDs are derived from apocrine glands, milk line, and epidermal cells. In many cases, ectopic EMPDs were noted on the milk line (e.g., breast, abdomen). Therefore, we speculate that ectopic EMPDs may be associated with accessory mammary glands.[Skin Cancer (Japan) 2023 ; 38 : 199-203]
