HPV type 35 is classified as a high-risk HPV type and is considered to be involved in tumor growth. We report the case of an 87-year-old woman who presented with a 6.5 × 4 cm erythematous, superficial papillary skin tumor in the perianal region. Histological examination revealed proliferation of abnormal keratinocytes with nuclear atypia in all layers of the epidermis. The tumor cells did not infiltrate into the dermis. Immunohistochemically, the tumor cells were positive for p16, and human papilloma virus (HPV) type 35 DNA was detected using HPV genotyping. Based on these findings, the patient was diagnosed with anal high-grade squamous intraepithelial lesion (HSIL). Palliative radiation therapy (39 Gy/13 Fr) was administered because the tumor was circumferential to the anus. The tumor flattened, and no complications occurred 6 months after the end of radiotherapy. Local excision is conventionally used to treat HSIL ; however, radiotherapy may also be useful for this condition.[Skin Cancer (Japan) 2024 ; 39 : 253-258]
