A male in his 60s was referred to our hospital for treatment of a 20 cm mass on his buttock. After skin biopsy, he was diagnosed with squamous cell carcinoma (SCC). Abnormally high white blood cell count and hypercalcemia were noted, further examination revealed that the tumor produced granulocyte-colony stimulating factor G-CSF and parathyroid hormone related peptide (PTHrP). The patient underwent tumor resection and right inguinal lymph node dissection, however, he developed pulmonary metastasis and local recurrence shortly after surgery. C’A’(CBDCA+Epirubicin) therapy was initiated and resulted in complete remission after 5 courses. Reports of tumors producing both G-CSF and PTHrP in cutaneous SCC are extremely rare. In this report, we describe a case of G-CSF- and PTHrP-producing SCC that progressed rapidly after surgical resection and responded well to chemotherapy, with a review of the literature.[Skin Cancer (Japan) 2024 ; 39 : 259-265]
