An 86-year-old male presented with an ulcer on the left temple and a mass on the right side of head. The left temple ulcer measured 27 × 23 mm, with an embankment-like border, and was associated with paralysis of the temporal branch of the left facial nerve. The mass on the right side of the head measured 22 × 18 mm. Both lesions were diagnosed as basal cell carcinoma based on skin biopsy. Echocardiography and MRI revealed that the left temple lesion had infiltrated the anterior auricularis muscle and temporal fascia. Total resection of the lesion, including part of the muscle and fascia, was performed. The mass on the right side of the head was resected simultaneously and skin grafting was performed for both lesions. Pathologically, the left temple lesion was diagnosed as morpheaform basal cell carcinoma with evidence of perineural invasion.[Skin Cancer (Japan) 2024 ; 39 : 293-298]
