A 78-year-old woman with a 10-mm brown mass lesion extending from the nasal apex to the nasal wing, and an 87-year-old man with a 12-mm brown mass lesion on the left nasal wing, both diagnosed as basal cell carcinoma through skin biopsy, were admitted to our hospital. In both cases, the lesions were excised under local anesthesia with a 3-mm margin, and surgery was completed with the application of an artificial dermis. After histopathological confirmation of negative margins, a composite tissue graft, including cartilage from the auricle, was performed as a two-stage reconstruction. Both patients had good results. In composite tissue transplantation, blood flow to the graft is limited because of the characteristics of the graft ; therefore, it is important to maintain blood flow to the graft. In the autopsy case, the skin defects were both less than 15 mm in size at the time of transplantation, which is regarded as a safe size for this procedure.[Skin Cancer (Japan) 2024 ; 39 : 287-292]
