We report a case of neurilemmoma arising in the masseter muscle. The patient was a 29-year-old man, who complained of a painless, elastic-hard mass in the right masseter region. MRI revealed a well-circumscribed mass, measuring 23×21×13mm, in the anterior area of the right masseter muscle. The lesion was of low signal intensity on T1-weighted images, and of high signal intensity on T2-weighted images. No regression was noted on fat-regression T1-weighted images. With a clinical diagnosis of benign tumor, he underwent excision of the lesion under general anesthesia. Histopathological diagnosis was neurilemmoma. The postoperative course has been uneventful with no recurrence for 3 years.
