A seventies-old woman was referred to our hospital with severe jaundice. Computed tomography showed enhancement and stenosis in the lower bile duct. ERCP revealed long stenose of both lower bile duct and pancreatic duct in the head of pancreas. Lower bile duct cancer was presumed and subtotal stomach-preserving pancreaticoduodenectomy was performed. Histopathological diagnosis was a well-differentiated adenocarcinoma in the lower bile duct. Although pancreatic invasion was seen in the surroundings of the pancreatic duct, direct invasion was not observed between the lower bile duct and the pancreas. We report a case of lower bile duct cancer with pancreatic invasion along the pancreatic duct toward the tail of pancreas.
