A case of surgically resected solid adenocarcinoma of the gallbladder with xanthogranulomatous cholecystitis successfully diagnosed after a period of observation

Abstract
A 68-year-old female with complaints of right hypochondralgia and appetite loss was referred to local hospital and consulted to our hospital for suspicious of gallbladder carcinoma. US image showed iso-echoic irregular shaped mass located in the neck and body of gallbladder and protruding to inner space. The tumor was enhanced gradually in the CT image and was iso-intensity in the T1WI and T2WI of MRI images. There was wall thickness with capsulized abscess located in the fundus of gallbladder. This lesion was improved after observation and thought to be of xanthogranulomatous cholecystitis (XGC). Cholecystectomy with partial hepatic resection and lymphadenectomy was performed. Macroscopically, nodular type milky colored tumor with expanding growth pattern and yellow-brown colored necrotic lesion were observed in the gallbladder. Microscopic findings were solid adenocarcinoma and XGC. It is often difficult to differentiate clinically XGC from gallbladder carcinoma. In this case, the diagnosis of XGC was made by the progress of time. Solid adenocarcinoma of gallbladder are rare tumor reported in the third edition of the Japanese classification of biliary tract carcinoma. A case of solid adenocarcinoma with XGC of gallbladder never reported.
