Abstract
A 65-year-old woman was admitted to our hospital because of abdominal pain. Abdominal ultrasonography and computed tomography (CT) revealed multiple gallbladder polyps, with situs inversus totalis. A laparoscopic cholecystectomy was performed for the definite diagnosis with some modifications. The surgical team changed sides with primary surgeon on the patients right. The trocars were inserted in the usual way but on the left side, and the operation was performed safely with both hands. The operation time was 94 minutes and the procedure was completed without complications except slight wound infection. Few cases of laparoscopic cholecystectomy in situs inversus totalis have been reported in Japan and the operative method has not been established. When we perform laparoscopic cholecystectomy with both hands in a patient with situs inversus totalis, left-hand manipulation is crucial, especially at the time of dissection of the Calot's triangle.
