A 78-year-old male with a 32-mm-wide solid and cystic lesion on his liver was referred to our hospital. Endoscopic retrograde cholangiography demonstrated a communication between the biliary tract and cystic lesion. Based on these findings, a clinical diagnosis of intraductal papillary neoplasm of the bile duct (IPNB) was made, and laparoscopic left hepatic lobectomy was performed. The pathological diagnosis of the tumor revealed an IPNB accompanying a microinvasive carcinoma. The pathological diagnosis of the tumor revealed a rare IPNB accompanying a microinvasive carcinoma.
